1、苯妥英钠(大仑丁)dilantin,phenytoinsodium：特点：作用较强;疗效高;为大发作首选，对精神运动性发作次之，对局限性发作也有较好疗效，但对小发作无效甚至恶化;无嗜睡作用;安全范围大;作用缓慢，口服一般需3～4天才显效，用于预防发作及维持治疗;而控制症状则以苯巴比妥为主。苯妥英钠抑制了na+内流，从而使细胞静息电位负值增大，加大与阈电位的距离，提高了脑细胞的兴奋阈，稳定膜电位，从而阻止了病灶放电的扩散。还能使脑中抑制性递质g-氨基丁酸的含量升高，这也与其抗癫痫作用有一定关系。苯妥英钠对三叉神经痛、坐骨神经痛及舌咽神经痛有止痛作用，可能与稳定神经细胞膜电位有关。对洋地黄中毒所致室性心律失常的疗效较佳。应引起注意的不良反应主要有：(1)齿龈增生：多见于儿童和青年，发生率约20%，因其抑制垂体-肾上腺系统，抑制acth、糖皮质激素分泌，使胶原组织增生。维生素c或保持口腔卫生可减轻此增生，停药3～6个月可消失。(2)长期服用可致佝偻病和骨质软化：药酶诱导剂可加速维生素d代谢，致缺钙。用d3和钙剂可预防和提高疗效。(3)久服骤停可引起发作，甚至诱发持续状态：(4)其它：略。

2、苯巴比妥phenobarbital,luminal：抑制大脑皮层运动区，提高惊厥阈，直接抑制病灶放电，又能限制放电扩散，使大发作脑电恢复正常。作用快，维时长(6hr)，毒性低，安全性较大，可作控制大发作首选;对小发作和精神运动性发作的疗效差。不可突然停药，长期应用可致成瘾。

3、扑米酮(去氧苯巴比妥，扑痫酮)primidone,mysoline：在体内转化为苯巴比妥和苯乙基丙二酰胺(pema)，对大发作、精神运动性发作及局限性发作都有较好疗效，但不如苯妥英钠。儿童对其有耐受性，故用量较大;体内消除较慢，长期应用有蓄积性;不可突然停药。
