羊癫疯发作的病理是什么

1.神经元痫性放电的发生：正常情况下，每一种神经元都有节律性的自发放电活动，但频率较低，一般为10～20Hz。在羊癫疯病灶的周围部分，羊癫疯病的发作病理让神经元的膜电位与正常神经元出现差异，在每次动作电位发生之后出现称为;阵发性去极化偏移的持续性去极化状态，并产生高幅高频(可达500Hz)的棘波放电。在历时数十至数百毫秒之后转入超极化状态。

2.放电的传播：当异常放电仅局限于大脑皮质的某一区域时，表现为部分性发作。若在此局部的反馈回路中长期传导，则导致部分性发作持续状态。羊癫疯病的发作病理通过电场效应及传播通路，也可扩及同侧其它区域甚至一侧半球，表现为杰克逊羊癫疯病发作。当异常放电不仅扩及同侧半球而且扩及对侧大脑半球时，引起继发性全身性发作。当异常电位的起始部分在中央脑(丘脑和上部脑干)而不在大脑皮质并仅扩及脑干网状结构上行激活系统时，则表现为失神发作;而广泛投射至两侧大脑皮质和网状脊髓束受到抑制时则表现为全身强直-阵挛性发作。

3.放电的终止：其机制未明，可能脑内存在主动的抑制机制。即在发作时，羊癫疯病灶内巨大突触后电位，通过负反馈的作用而激活抑制机制，使细胞膜长时间处于过度去极化状态，抑制放电过程的扩散，并减少病灶的传入性冲动，促使发作放电的终止。此外在此过程中，抑制发作的代谢产物的积聚，神经胶质细胞对钾及已经释放的神经介质的摄取也起重要作用。
