什么是病灶性癫痫

【一】痫灶如何形成呢。

随着刺激不断重复，与刺激局部相关联的投射区域逐渐扩大且包含了很多先前不曾涉及到的区域，这样一个多灶性结构和多网络性分布趋势，使大鼠更容易对相应的病生理刺激做出惊厥反应，而产生更明显的临床表现。我们对远隔部位的双侧海马区进行了病理检查，结果发现:刺激4周时，大鼠的海马就有了组织学的改变，主要表现为海马的水肿和空泡样变性，尚未见神经元坏死及数目减少。

随着刺激时间的延长，大鼠的海马神经细胞数目减少，且出现细胞变形，核固缩，尼氏体崩解或消失，胞浆呈嗜酸性均一着色等改变;刺激12周时，神经细胞明显坏死数目减少，大部分为变性的红色神经元及增生的神经胶质细胞，海马的正常复层结构消失。这一病理组织学变化充分证明癫痫灶的形成有一个由弱渐强的过程，早期是可逆的，晚期则不可逆。这一现象符合临床规律，提示某些癫痫发作愈早治疗预后愈好。

【二】癫痫灶的发展

许多研究己证明，引起全面性癫痫发作的功能解剖区相对确定，它是由于丘脑一皮层回路受到干扰所致，发作的行为学表现不仅与刺激部位有关，还依赖于脑的某些特殊的神经核团。癫痫灶的形成和发展过程中，海马的记忆功能发挥了十分重要的作用。

临床实践中，我们亦可见到许多皮层受损的癫痫患者在海马切除后其痛性发电消失，组织学检查可见明显的海马硬化等现象，与组织学改变相一致的是脑电功能学方面的改变，实验过程中我们定期对大鼠进行了脑电监测，随着刺激时间的延长，动物在实验不同阶段出现了不同的脑电表现，爆发性棘或尖慢综合波是TPS发作时的明显脑异常放电的特征，后放则是点燃过程中的另一种重要的表现形式，一旦动物有过点燃，对刺激的应答就会增加，并在发作间期脑电图有自发的癫痫样放电。
