1、先兆期：约有14%的患者有先兆，在意识丧失前感到胃部不适、头晕等。

2、强直期：患者突然意识丧失，跌倒在地，全身肌肉强直性收缩，头向后仰，双上肢屈曲强直，双下肢强直伸直。由于膈肌和肋间肌强直收缩，将肺内空气压出，而喉头肌肉此时也痉挛狭窄，使空气通过时发出一声吼叫，因此，俗称“羊角风”。此时瞳孔散大，对光反射消失，呼吸暂停，面色青紫，持续约数十秒。此期因突然意识丧失、昏倒，可发生跌伤或其他意外。

3、阵挛期：此期全身肌肉发生有节律的抽动。由于咀嚼肌抽动而常将舌咬破，口吐白沫。四肢抽动先快而小，继之慢而大，最后停止，持续l~3分钟。可伴有大、小便失禁。

5、恢复期：阵挛停止后，患者仍处于昏睡状态，经十多分钟至半小时后意识逐渐恢复。清醒后，患者对发作过程不能回忆，自觉头痛乏力，肌肉酸痛。有的患者在恢复期出现兴奋躁动，乱跑乱叫，甚至打人毁物等，称癫痫后精神障碍。
