老年人的身体毕竟和普通人不一样，老年人抗癫痫药物的敏感性增加、肾小球的滤过下降，药物在体内的存留时间更长，所以采用低于平均剂量也会收到很好的疗效。同时需要选择尽可能不通过肾脏滤过的药物。老年癫痫患者服用有酶诱导作用的药物如丙戊酸钠更易发生骨质疏松症。由于老人可能合并其它的内脏疾病，需要同时服用其它药物，所以在癫痫药物选择上尽可能避免药物间的相互作用，所以要尽可能选择类似单一抗癫痫药物治疗。
