1.癫痫持续状态的保持呼吸道通畅

病情发作时取头低侧卧位或平卧位,解开衣领,头偏向一侧,及时吸出口咽部分泌物和呕吐物,防止窒息和肺部感染,发作的间歇,如有舌根后坠阻塞呼吸道,可用舌钳或置口咽通气导管。分泌物聚集堵塞呼吸道并发感染或窒息,可行气管插管,持续吸氧。抽搐时加大氧浓度达50%左右,密切观察呼吸频率,节律,深度变化,听诊肺部体征,监测氧饱和度,对出现呼吸过慢或氧饱和度<85%,给予呼吸机辅助呼吸,必要时行气管切开。本组有5例气管插管者呼吸机辅助呼吸后氧饱和度仍不能上升到90%而行气管切开,其中2例因呼吸衰竭死亡。

2.避免癫痫持续状态的外伤

此病发病急,意识丧失突然,易造成自伤或他伤,因此须做好保护。患者发作时立即采取平卧位,头偏向一侧,松开衣领、腰带,取出假牙,用裹有纱布的压舌板,紧急情况下可用折叠成条状的小毛巾或手帕置入患者的上下臼齿之间,以防抽搐咬伤舌,颊。抽搐时用手托患者下颌,减少上下牙齿咬动,防止下颌关节脱臼,加床档或使用约束带适当保护,不可强行按压肢体,以免造成肌肉,关节的人为损伤或骨折。

3.加强癫痫持续状态的基础护理

做好健康教育保持患者病房周围安静,避免声光的刺激,加强安全及口腔.皮肤的护理,病情稳定.神志清醒后应鼓励患者树立战胜疾病的信心。积极、耐心、持久的配合治疗,在医生的指导下服药,不乱用药物,生活规律,防止精神刺激及感情冲动,勿从事有危险的工作,定期复查,观察用药效果。
