失神发作以意识障碍为主，其特点是没有任何先兆的、突然开始的发作。患者正在进行着的动作突然中断，发呆，还可伴有双眼上翻。患者如果在讲话，其言语会变慢或终止;如果在走路则可能突然站立不动、呆若木鸡，也可能突然挣脱大人搀着的手而向前快走几步，又突然回头哭着找其父母;如果在进食时，夹着食物的筷子在送往口里的中途突然停住，对问话不能作答，某些患者当人们与其说话时可使发作终止。该种发作持续数秒至30秒，超过一分钟者少见。

典型的失神发作也称小发作，表现为突然发生和突然休止的意识障碍，一次持续5～30秒，事后对发作全过程无记忆。脑电图在发作中呈现双侧对称的2.5～4hz棘―慢波，偶尔为多棘―慢波，发作间期也可有同样或较短的阵发活动，背景波形正常。可以从儿童期持续至成年期。

失神伴意识障碍：表现为仅有意识障碍，发作时正在进行的动作突然终止，主要以突然发生和突然停止的发呆、双眼凝视为特点，还可有动作停止、持物失落，既无先兆又无回忆，来得快去得也快，患者不能了解发作的经过，但发作前所做的动作仍可继续，如不注意难以发觉这种发作。

失神伴轻度阵挛的成分：发作开始完全同仅有意识障碍者，但尚有眼睑、口角或其他肌群的颤抖，由不易觉察一直到全身性肌阵挛。眼睑的阵挛性发作可导致节律性睡眠。阵挛往往为双侧性，但一般不影响其姿势，如手中持物则可能掉落。

失神伴无张力成分：表现为除失神外的因低张力所产生的头或躯干向前倾，伴有头部、上肢的下坠;手中所拿物件掉落;偶尔出现因全身肌张力丧失而致患者跌倒;若维持姿势的肌张力减退不对称，那么头可垂向一侧，躯干倾斜;如果患者站着双膝可弯曲。
