单凭脑电图异常不能诊断癫痫病因，癫痫患者的发作间歇期脑电图异常可分为特异性异常和非特异性异常。所谓非特异性异常，如局限性或弥漫性慢波，可见于脑部各种器质性病变。这种脑电图当然不是诊断癫痫的可靠证据，因此不能单凭脑电图显示的结果来进行判断。

当诊断时脑电图出现了一些特异性改变如尖波、棘波、尖一慢波、棘一慢波时，可不可以不论临床有无癫痫发作就诊断为癫痫呢?也不行。有癫痫发作的患者，脑电图描记时出现上述的典型癫痫波，可以确诊该患者患癫痫，而从来没有癫痫发作的人脑电图即使出现典型的癫痫波，也不能据此诊断某人是癫痫患者。这是因为少数正常人，尤其是有癫痫家族史者和儿童，脑电图有时可出现典型的癫痫波，但他们从来没有癫痫发作，所以不能因为一张脑电图就随便的妄加判断。

这些人目前还没有发生什么危险，将来发生癫痫的危险性如何?为解决这个问题，有人在一大批学龄儿童中发现有千分之几的正常儿童脑电图有典型癫痫波，经过若干年的追踪观察，这些儿童发生癫痫的比例并不比当初脑电图正常的儿童大。因此可以认为，单凭脑电图上的癫痫波既不能诊断癫痫，也不能预示将来一定会发生癫痫，因此不能单凭脑电图来预测。
