3、在添加治疗时，应缓慢将已有不良反应的第1种药物减至中等剂量，之后再添加不同药理机制的另1种药物。两种药物的联合应用疗效似乎优于先后替换两种药物的单药治疗。如果患者两种药物的联合应用在3月内仍未见效，应缓慢转变为第2种药物的单药治疗，并开始添加新选择的抗癫痫药物。

4、对痫性发作应明确是事件相关性发作，还是内科疾病引起的抽搐，或是很少发作仅表现为轻度抽搐的良性癫痫。针对内科疾病引起的痫性发作，应针对原发病进行治疗，仅仅是在患者反复抽搐、诱发因素不能纠正时才需要抗癫痫药物长期治疗。失神性癫痫状态或非惊厥性简单部分性癫痫状态，没有脑损伤的肯定证据，且预后较好，一般不需要过激的抗癫痫药物治疗。非痫性发作的误诊常引起不恰当的抗癫痫药物治疗。
