是按临床发作症状表现区分为不同类型的癫痫，如大发作、小发作、局限性发作、植物性发作、精神运动性发作等。将上述两项分类依据结合起来，是临床上一种简单的癫痫分类法，即在临床发作类型之前冠以原发性或继发性，如原发性癫痫大发作。

癫痫病分类标准三

是按脑电图描记的图形进行分类。如1934年在英国伦敦为纪念jackson诞辰100周年纪念日举行的国际神经学会、国际癫痫学会上，lennox报告癫痫失神发作的病人约90%在发作时或者间歇期有3次/秒的综合棘慢波，升命名为失神发作。此后，引起学者对癫痫脑电图描记的极大重视。有人通过一些资料说明脑电图描记对癫痫的诊断，有4种类型可以明显地区分出来，即大发作、小发作、小发作变异、精神运动发作。这种分类方法，从诊断的角度来看具有重要意义，但从疾病分类学的观点看，则意义不大。因为临床所见的发作类型与脑电图描记波型之间并无完全相应的关系。尽管应用脑电图描记作为癫痫的分类仍存在很多不足之处，但仍被大多数学者所接受。
