一、癫痫伴觉醒期大发作的病因

癫痫伴觉醒期大发作为特发性癫痫综合症。对发生在睡眠中、醒觉状态和觉醒期的研究显示，觉醒期的患者大多数无病因可寻。EGMA患者家族中有热性惊厥或癫痫者比发生在睡眠中和清醒状态的大发作患者明显增高，而与儿童及少年失神性癫痫、少年肌阵挛性癫痫等其他特发性全身性癫痫相似，提示本症的发病与遗传性惊厥易感性有关。

除内在的遗传易感性外，一些外在的因素对EGMA的发作具有诱发或易化作用。最重要的诱发因素为睡眠缺乏，如剥夺睡眠或慢性睡眠不足。多导睡眠图研究显示，EGMA患者的睡眠特别不稳定，易受外界因素的影响。此外，光刺激、疲劳、酗酒等也容易诱发发作。

二、癫痫伴觉醒期大发作的临床表现

癫痫伴觉醒期大发作以男孩多见，发病率在62.2%～65.6%。起病年龄范围在6～35岁，多数在10～20岁起病，平均为17.1岁。几种已被认识的特发性年龄相关性全身性癫痫综合症的发病年龄互有重叠，癫痫伴觉醒期大发作是其中的最后一个高峰年龄组。

发作表现：患者的多数发作在时间上和睡眠一觉醒的状态变换有关，而不在于是发生在日问还是夜间。日间睡醒、夜间短暂觉醒及清晨觉醒均可发作，常出现在醒后1小时左右的短时间内，为典型的GTCS。病人可以仅有GTCS一种发作类型(单纯型)，也可合并典型失神发作、肌阵挛发作及局部性发作(联合型)。在某些EGMAr患儿，GTCS之前常有意识清楚的连续肌阵挛性抽动(肌阵挛持续状态)或持续的意识朦胧(失神持续状态)，并最终以GTCS结束发作;也有少数病人在GTCS后出现失神持续状态。
