起病年龄,儿童时期癫痫起病越早，成为难治性癫痫的可能性越大。新生儿开始的癫痫，约1/2预后差，约1/4死于发育畸形、感染等。婴儿早期癫痫性脑病，(大田原综合征)多在出生后前3个月发病，发病年龄早是大田原综合征的主要特征。婴儿痉挛症发病在1岁以内，Lennox-Gastant综合征多在2-5岁内发病。而在其他类型的癫痫中，首发年龄越大，成为难治性癫痫的可能性越大，尤其在超过60岁以上的老年人往往对常用的抗癫痫耐。

癫痫状态与初期治疗效果,由于癫痫状态可引起脑部损伤和改变中枢神经系统的可塑性，因而有过癫痫状态，尤其在以癫痫状态为首先症状的癫痫人，易成为难治性癫痫。难治性癫痫与发作类型有关。典型失神和单纯大发作预后较好。大样本病例调查结果发现，复杂部分发作约40%成为难治性癫痫。婴儿痉挛症和Lennox-Gastant综合征治疗往往困难，常遗留智力不全。
