脑外伤癫痫可以通过哪些药物治疗呢

脑外伤癫痫的特征:脑外伤的部位-如贯通性损伤、复杂的神经外科手术、占位性损伤、皮质挫伤、凹陷性骨折。潜伏期较长-颅脑损伤后发生过早期癫痫的病人中发生晚期癫痫的风险是无早期癫痫发生者的2.5倍。损伤部位与癫痫的发病率-颅脑任何部位损伤都可引起癫痫，但额叶后部、顶叶和弥漫性脑皮质损伤所引起的癫痫明显高于其他部位。对重度脑损伤患者，如硬脑膜完整，则癫痫的发生率为7%～39%;如硬脑膜有破损，则为20%～57%。

脑外伤癫痫的药物治疗：

(1)苯妥英:苯妥英是最常用的抗癫痫药物。临床上应缓慢给药，给药的速率应限制在50mg/min以下，以避免注射部位的疼痛及降低心血管的毒性。苯妥英仅可以加入生理盐水中，并避免从同一根输液管中给予其他药物。

(2)戊巴比妥:可以在停药后很快恢复意识。通常会出现呼吸抑制和低血压等副作用。推荐的静脉注射剂量为第1小时内给予6～15mg/kg，之后予每小时0.25～2.0mg/kg。戊巴比妥与其他药物相比，较少出现短期治疗失败和爆发性发作。

(3)咪达唑仑:咪达唑仑是一种高度亲脂药物。肌肉注射咪达唑仑不需要开通静脉通道，终止发作起效较快。

(4)普鲁泊福:普鲁泊福是一种非巴比妥类麻醉药，起效快，已用来控制癫痫发作。

(5)其他:除传统的抗惊厥镇静药物用于治疗癫痫外，最近几年对许多新的抗癫痫药物进行了大量的研究，比如对磷苯妥英、丙戊酸、左乙拉西坦、托吡酯、加巴喷丁、拉莫三嗪、唑尼沙胺、奥卡西平、氨己烯酸等。所有的研究都表明，这些药物是有效的，且可以用来治疗难治性癫痫发作。因此临床上可以在传统抗癫痫药物使用的基础上适当的增加这类新药的使用，以期增强疗效，获得更好的结果。
