24小时脑电图说明是什么?棘波为突发性的一过性脑电图变化，明显突出于背景，乃痫样波形是最具特征性的表现之一。棘波的出现提示脑部有刺激性病灶。若在慢波背景上出现的棘波，常揭示来自原发癫痫灶或其附近区域，而在正常背景上出现棘波，波幅常较低，周期长，多由远处病灶传播而来。若在脑电图描记中出现棘波逐渐增多现象或形成棘节律，常预示即将出现临床发作。棘波可见于各型癫痫。

尖波为突发性一过性脑电图变化，意义与棘波相同，乃神经元同步放电的结果，为常见的痫样波的特征波形之一。它由较大的癫痫灶中多数神经元棘波放电的不完全同步，或为较大的病灶中大量神经元同步性放电恢复的延迟，也可以是病灶在皮质深部或皮质下棘波灶远距离传播，为棘波时间上的延长。可见于各型癫痫发作间期脑电图中。棘-慢复合波这种异常放电可能起源于皮质深部的中线结构，或始于视丘，而影响的皮质只限于背内侧核的投射部分。复合波中慢波是主要成分，比较规则而有节律，棘波出现其间，若在慢波的上升支或下降支上，波幅高低不一，一般不超过慢波波幅。

尖-慢复合波乃尖波后跟一个慢波。出现形式多种多样，多呈不规则同步爆发，也可见弥漫性或连续性出现，亦可一侧性或局限性出现。多见于颞叶癫痫，弥漫性尖-慢节律见于顽固性大发作和失动性小发作，提示脑组织深部存在较广泛的癫痫病灶。是由数个棘波和一个慢波组成，常成串连续出现或不规则出现。棘波波幅高低不一，但一般不超过慢波。常预示有痉挛发作，是肌阵挛性癫痫最具特征的波形之一。为2～6个棘波成簇单独出现。有时也随附一个或多个慢波，形成多棘-慢复合波，多见于肌阵挛性癫痫，是多棘-慢复合波的一种变异形式。但当棘波连续出现，数量增加，频率变快，或由某一脑区逐渐扩散至整个大脑时，则预示患者将出现临床发作，或系发作开始的脑电图表现，且多为大发作。

亦称发作性节律波。即于原有脑电图背景上出现阵发性高波幅节律。其成分为δ节律、θ节律、α节律和β节律，多呈高波幅发放，明显区别于背景脑电图，突然出现，突然消失。多源于中央脑系统病灶发出，但诊断价值不如上述放电波形。见于3个月至5岁婴幼儿，以3-8个月最常见。最多见于婴儿痉挛，预示患儿存在严重的脑损伤。
