目前的EEG机记录到的波形是放大了100万倍后的结果。EEG可用波型、波幅、频率及位相来表示。当脑出现病理性或功能性改变时，EEG就会发生相应变化。因为在癫痫发作时必定有异常放电，而在癫痫发作间期也可记录到异常放电。

脑电图是成人癫痫诊断最有效的辅助诊断工具，如能结合多种激发方法(过度换气即大喘气，闪光刺激，药物，睡眠等)以及结合特殊电极，至少可在80%患者中发现异常癫痫样波。脑电图检查可帮助确定是否患有癫痫，对临床诊断困难的非典型癫痫发作和癫痫分类，脑电图检查有时起着决定性作用。脑电图检查对于确定癫痫病灶有重要意义，对于一些没有形态学改变的癫痫灶，CT，MRI可能无异常表现，有时主要靠脑电图来定位。

据统计80%左右的成人癫痫患者都有脑电图异常，而只有5%～20%的癫痫患者发作间歇期脑电图可表示正常。若能重复检查，使用适当的诱发试验和特殊电极，其阳性率可达90%～95%。故EEG检查对成人癫痫的诊断、定位定性、判断类型及疗效观察，都具有十分重要的意义。凡在脑电图上出现棘波、尖波、棘慢波、尖慢波及多棘慢波，统称痫样波，也称痫性放电或痫样波发放，亦可称发作波。
