①大发作选用苯巴比妥90-300mg/d。丙戊酸钠0.6-1.2/d,卡马西平600-1200mg/d等。

②复杂部分性发作：苯妥英钠0.2-0.6/d,卡马西平0.2-1.2/d。

③失神发作:氯硝安定5-25mg/d,安定7.5-40mg/d。

④癫痫持续状态:首选安定10-20mg/次静注。

2.药物剂量从常用量低限开始,逐渐增至发作控制理想而又无严重毒副作用为宜。

3.给药次数应根据药物特性及发作特点而定。

4.一般不随意更换或间断，癫痫发作完全控制2-3年后,且脑电图正常,方可逐渐减量停药。

5.应定期药物浓度监测,适时调整药物剂量。

二、在癫痫发作的治疗中，抗癫痫药物有特殊重要的意义

抗癫痫药物可通过两种方式来消除或减轻癫痫发作，一是影响中枢神经元，以防止或减少他们的病理性过渡放电;其二是提高正常脑组织的兴奋阈，减弱病灶兴奋的扩散，防止癫痫复发。

一般将60年代前合成的抗癫痫药如：苯妥英钠、卡马西平、乙琥胺、丙戊酸钠等称为老抗癫痫药，其中苯巴比妥、苯妥英钠、卡马西平、丙戊酸钠是目前广泛应用的一线抗癫痫药。

但有些发达国家，由于苯巴比妥、苯妥英钠的一些副作用，已将其列入二线抗癫痫药。仅将卡马西平、丙戊酸钠列为一线抗癫痫药。新的抗癫痫药如：加巴喷丁、拉莫三嗪、氨已烯酸、托吡酯等，目前比较新的是优时比的左乙拉西坦片。

三、对于明确病因的癫痫，除有效控制发作外要积极治疗原发病

四、对药物治疗无效的难治性癫痫可行立体定向术破坏脑内与癫痫发作的有关区域
