经过调查老年人癫痫多为继发性，所以病因治疗很重要。随着基础病因的消除，癫痫发作亦多会有所改善，脑血管病继发的癫痫，特别是脑梗死急性期发生者提示预后不良，如病情能稳定好转，癫痫发作控制后，可以逐渐停用抗痫药物。老人可能合并其它的内脏疾病，需要同时服用其它药物，所以在癫痫药物选择上尽可能避免药物间的相互作用。

在治疗时老年人癫痫要坚持长期系统用药，癫痫病治疗是一个长期过程，患者和家属对此应有充分的心理准备，老年人癫痫病应在医生指导下确定是否开始用药。偶尔一次癫痫发作并不一定需要用抗癫痫药，如一年中有两次或多次发作，可根据具体情况酌情用药治疗。
