指反复发作并以一定的姿势造型为主要癫痫的症状表现形式，常因额叶辅助运动区受累所致。这类典型的癫痫症状表现为头眼向一侧偏斜，伴一个或多个肢体姿势性强直，常见面向侧的上肢上举，肘部半屈，似击剑状，同时意识多保留，部分有尖叫或不能说话。整个发作过程持续时间短。这种姿势性发作一般不伴有阵挛性动作，但可向其他形式的发作转化。

特殊癫痫病之二：癫痫性眼球震颤

以眼球震颤为主要或唯一的癫痫症状。中医传统上把引起患者眼球震颤的原因称作痫证或痫病，癫痫是后来的叫法。作为局灶性癫痫症状的一种表现，癫痫性眼球震颤是罕见的。患者以前可无癫痫病史，癫痫性眼球震颤可以毫无诱因而突然发作，也可以在某些特定因素，如光刺激、深呼吸、压迫眼球等情况下发生，眼球震颤多为水平性，快相向癫痫灶对侧，可伴有背离或朝向癫痫灶的水平性凝视。癫痫发作时患者可能有视物不清感，持续时间短暂。脑电图检查放电部位多在颞顶枕部。

特殊癫痫病之三：旋转性癫痫病

旋转性癫痫病是癫痫的一种特殊类型，临床易与扭转痉挛及其它一些精神性疾病混淆，造成误诊。本病临床较少见，以反复的转动性发作为主要癫痫症状表现。癫痫发作，可表现为单纯的头眼突然转向一侧，大多转向异常放电的对侧，也可伴有躯干的转动，患者似向侧或后看。偶尔可造成全身旋转，其幅度可多达几圈。发作一般历时短暂，意识清醒者可主动扶持物体以防跌倒，部分于转圈后跌倒于地，继以全身强直-阵挛发作。
