多数癫痫病人的脑组织存在有病理改变，即所谓“癫痫病灶”。在“癫痫病灶”中，神经病理改变主要为限局性硬化、限局性瘢痕(感染或外伤后)、脑膜粘连、新生物等导致局部脑组织崩解，供血障碍，神经细胞外液成分改变，组织结构发生紊乱，从而使细胞的生理功能、生物化学结构和新陈代谢等发生异常，给局部细胞电生理改变打下基础。

典型的大脑皮层癫痫病灶有中心区和环形中间带，代之以瘢痕组织和胶质细胞增生，已不具有神经的兴奋性，所以也没有电活动。环形中间带的神经细胞数目减少，并由于供血不足及代谢紊乱而发生变性，具有过度兴奋性，有异常的电活动，是引起癫痫放电的区域。其异常放电通过周围正常脑组织而传播，这就构成了癫痫发作的病理基础。

大脑神经元的功能是产生和发送神经冲动，神经纤维的任务是传送神经冲动。神经冲动的传导是通过一个神经细胞的轴突与另一个神经细胞的树突和胞体互相接触，形成所谓“突触”来实现的。每一树突受体的突触可达数千个。神经元末梢的囊泡释放神经递质，通过突触间隙，作用于另一神经元的胞体和树突的受体，改变突触后膜的极化状态，使其发生兴奋性或抑制性变化。
