控制癫痫的药物对小儿的副作用

与药物剂量有关的不良反应：比如苯巴比妥、氯硝西泮引起镇静，苯妥英钠和卡马西平引起头晕和平衡困难。为了减轻这类不良反应，要注意从小剂量缓慢加量，逐渐达到起效和维持剂量。使用过程中要监测血药浓度，绝对不能超过说明书推荐的最大治疗剂量。采取这些措施后，上述不良反应大多可以避免发生或逐渐减少、最终耐受。

由药物长期应用引起的不良反应：比如长期服用丙戊酸钠，可以引起多囊卵巢综合征和肥胖。长期服用苯妥英钠，可以导致牙龈增生、多毛、骨质疏松等。对这些不良反应，一方面某些特定人群要避免使用某种药物，比如青少年女性、育龄妇女和肥胖症患者要尽量不用丙戊酸钠。另一方面，服用这些药物后，尽量维持在可以控制发作的最小剂量，并且在无发作持续足够多年后尽量停药，要谨记药物治疗癫痫病的事项。

与患者自身的特异体质有关的不良反应：比如对卡马西平、奥卡西平、拉莫三嗪过敏的患者可能出现剥脱性皮炎等严重的皮肤损害，对丙戊酸钠不耐受的患者可能出现严重的肝损害等。这类不良反应发生率极低，但是一旦发生对患者的影响很大甚至致命。
