临床研究发现，癫痫性痉挛可以累及全身肌群，也可以累及节段性颈屈肌腹肌或肩部肌肉，肌肉收缩强度通常较大，但也可以为轻中度，痉挛后继以长达九十秒的运动减少和反应降低期，但在极少数情况下，整个发作完全表现为运动减少和反应降低。治疗癫痫病痉挛期常常出现呼吸改变或呼吸暂停，但是心率改变罕见。发作终止后常伴有哭泣或大笑。

个体患儿会出现下列一种以上的痉挛发作形式，同时也受到体位的影响。屈肌和伸肌同时受累多见，也可仅累及屈肌，伸肌单独受累少见。屈肌痉挛常见被形象的描述为点头状痉挛，折刀样痉挛以及闪电样点头状痉挛。治疗癫痫病屈肌痉挛表现为颈躯干突然屈曲手臂前伸或侧举，有时伴屈肘，下肢呈屈髋屈膝状到底干过伸，肢体外展伸直，类似于moro反射。

屈肌伸肌痉挛是最常见的癫痫病的症状是屈肌和伸肌突然同时收缩，颈躯干及上肢屈曲，但下肢伸直。
