第一阶段，癫痫病治疗是要从患者的首次发作开始的。

因为患者的首次发作可能掺杂着其他的发作环境，且还有部分癫痫患者是单次非热性发作，所以可能不会引起注意。不过，约一半以上患者会在首次发作后的1个月内复发，即出现第二次发作，此时基本上将诊断为癫痫，并开始治疗过程。

第二阶段，基本上是指患者首次发作后的数年。

这期间，约七成的患者通常在较短时间内进入持久的缓解，只有少数复发。经过这个阶段的治疗，绝大多数患者能够停药，预后良好直至痊愈。而剩下的三成患者则会出现发作持续无缓解，醉成发展成潜在的慢性顽固性癫痫，这种癫痫类型的预后一般都会相当差。

第三阶段，也就是上一阶段的那三成的患者。

他们或者是慢性顽固性癫痫，或者是终生缓解，即通过药物等相关治疗，其中的20%的患者最终缓解。不过专家介绍，鉴于癫痫发作对患者神经系统的损害，致使缓解的可能性会随着活动时间的正常而减小，且复发率也会随之升高。
