大发作又称全身强直-阵挛发作，发作开始后病人失去知觉，有时会先尖叫一声，然后全身僵硬，继而四肢有节律地抖动。有时会暂停呼吸而面色青紫。可能会咬破舌头，大、小便失禁。大发作通常持续1～3分钟，在恢复知觉后，患者可能有剧烈头痛或入睡。小儿患者常很快恢复到正常状态。

强直性发作，表现为全身僵硬，可能突然跌倒，四肢没有节律性抖动，也可有手上举，头后仰，或头转向一侧，伴面色或口唇青紫。阵挛性发作表现为四肢、头面部有节律抖动，而不会身体长时间僵硬不动。失张力发作表现为病人突然肌肉放松，不能维持当时的站立或坐位姿势，跌在地上，不醒人事，有时仅表现为低头，两肩下垂，手指张开，不一定跌倒。失神发作表现为突然不动，但不跌倒，两眼发直，发呆，凝视前方。有时眼皮快速跳动或手微微抖动。一般10秒钟左右恢复正常，自己无知觉。失神发作每天可能发作数次到几十次，可达1天上百次。失神发作不像其它类型发作那样引人注目，容易被忽视。家长常反映孩子学习成绩特别是数学成绩下降。或上课、吃饭时发呆，同伴反映玩游戏时突然莫明其妙停下来，几秒钟后又自动恢复正常。

1.羊角风西药治疗

①大发作选用苯巴比妥90-300mgd。丙戊酸钠0.6-1.2d,卡马西平600-1200mgd等。

②复杂部分性发作：苯妥英钠0.2-0.6d,卡马西平0.2-1.2d。

③失神发作氯硝安定5-25mgd,安定7.5-40mgd。

④癫痫持续状态首选安定10-20mg次静注。
