癫痫病虽然治疗方法颇多，但是癫痫药物是主要治疗手段，绝大多数患者可通过癫痫药物治疗得到控制。药物治疗癫痫的首要目标是控制发作，包括减少发作次数和减轻发作程度。在控制发作的同时，还应注意改善患者的生活质量。尤其是难治性癫痫患者治疗的目标应定位在控制发作和减少不良反应，尤其是在运动功能改善及提高生活质量上找到最佳点，所以患者不要放弃希望。

对于比较严重的癫痫患者可以选择手术治疗，如果癫痫发作无法用抗癫痫药控制，应行癫痫外科手术治疗。对于顽固性癫痫的评定建立于患者对药物不良反应耐受的基础上，当治疗药物浓度已给患者带来严重的不良反应时，应考虑手术治疗。手术治疗癫痫无最小年龄限制。一些发达国家对于婴幼儿难治性强调早期手术，但是这并不是说这时候手术就没有危险了。

“手术有危险，治疗需谨慎”，儿童癫痫手术方法有颞叶切除术、选择性海马杏仁核切除术、半球切除术、胼胝体切断术、多处软脑膜下横纤维切断术、脑皮质癫痫灶切除术、迷走神经电刺激术治疗顽固性癫痫等。癫痫手术患者的选择应符合：必须是药物难治性癫痫;必须存在明确的致痫灶;手术必须不会引起严重功能障碍;对于存在脑结构性病变的患者，无须满足难治性癫痫的标准，可手术者应尽早手术;对于年幼患儿，原则上应该早期手术，但是这选择也要慎重。
