什么是全面性癫痫病

全面性癫痫具有全面性发作，即发作起始双侧大脑半球同时受累。发作期的脑电图模式开始即为双侧性。

l、特发性2、隐源性3、症状性(l)无特异性病因(2)有特异性病因

1.特发性全面性癫痫所有的发作都是全面性的，并且脑电图也是全面性双侧同步对称的异常放电。患者在发作间歇期往往正常，缺乏神经系统以及神经放射学的阳性发现。而发作间歇期脑电图背景活动正常，但可以出现全面性癫痫样放电，例如棘波、多棘波、棘慢波和多棘慢波等(参见发作类型分类中相对应的放电模式)，放电频率大约33Hz，并在NON—RE141期增多。不同的特发性全面性癫痫综合征有不同的疾病好发年龄。

2.症状性全面性癫痫大多数发生在婴儿以及儿童期。以全面性发作为特征，但是临床和脑电图表现不同于特发性全面性癫痫类型。症状性全面性癫痫临床可能仅有一种发作形式，但是往往有多种严重的发作类型，包括肌阵挛发作、强直发作、失张力发作以及非典型失神发作等。脑电图表现为双侧性，但是放电的节律性较特发性类型不规则，并且多多少少有些双侧不对称。发作间歇期的脑电图可以表现为抑制爆发模式、高幅失律、慢的棘慢波综合或者全面性快节律，局灶性异常可以伴随上述的任何全面性异常模式。临床表现、神经心理学和神经放射学常常提示弥散性的特异或者非特异性脑病。
