(一)药物治疗，合理使用抗羊痫风药物是当前小儿羊痫风的治疗的主要手段。

抗羊痫风药物使用原则遵从以下原则是实现合理用药的基础。

(1)早期治疗：反复的羊痫风发作将导致新的脑损伤，早期规则治疗者成功率高。但对首次发作轻微，且无其他脑损伤伴随表现者，也可待第二次发作后再用药。

(2)根据发作类型选药：常用药物中，丙戊酸(VPA)与氯硝基安定(CZP)是对大多数发作类型均有效的广谱抗羊痫风药，而抗羊痫风新药中，主要是妥泰(托吡酯，TPM)和拉莫三嗪(LTG)有较广抗羊痫风谱，注意药物剂量。

(3)单药或联合用药的选择：近3/4的病例仅用一种抗羊痫风药物即能控制其发作。但经2～3种药物合理治疗无效、尤其多种发作类型患儿，应考虑23种作用机理互补的药物联合治疗。小儿羊痫风的治疗需要耐心持久，切忌私自断药。

(4)用药剂量个体化：从小剂量开始，依据疗效、患者依从性和药物血浓度逐渐增加并调整剂量，达最大疗效或最大血浓度时为止。一般经5个半衰期服药时间可达该药的稳态血浓度。同时注意患儿的体重，根据患儿的体重来调节药量。

(5)长期规则服药以保证稳定血药浓度：一般应在服药后完全不发作2～4年，又经3～6月逐渐减量过程才能停药。不同发作类型的疗程也不同，失神发作在停止发作2年，复杂性局灶性发作、LGS等则要停止发作后4年考虑停药。婴幼儿期发病、不规则服药、EEG持续异常、以及同时合并大脑功能障碍者，停药后复发率高。青春期来临易致羊痫风复发加重，故要避免在这个年龄期减量与停药。

(6)定期复查：密切观察疗效与药物不良反应。除争取持续无临床发作外，至少每年应复查一次常规EEG检查。针对所用药物主要副作用，定期监测血常规、血小板计数或肝肾功能。在用药初期、联合用药、病情反复或更换新药时，均应监测药物血浓度。

(二)手术治疗，临床中对于小儿羊痫风人不常用手术治疗，约有20%～25%的患儿对各种抗羊痫风药物治疗无效而被称为难治性羊痫风，对其中有明确局灶性羊痫风发作起源的难治性羊痫风，针对于这种羊痫风，可考虑手术治疗。近年对小儿难治性羊痫风的手术治疗有增多趋势，其中2/3因颞叶病灶致羊痫风难治而行病灶切除，术后约52%发作完全停止，36%有不同程度改善。其他手术方式包括非颞叶皮层.区病灶切除术、病变半球切除术，以及不切除羊痫风灶的替代手术(如胼胝体切断术、软脑膜下皮层横切术)。做好术前评估是决定术后疗效的关键。
