★精神异常：有些癫痫病人表现为情感淡漠、表情呆滞、懒言少动、反应迟钝、思维迟缓、情绪不稳、易激怒、行为异常、凶残。以上精神障碍多见于脑器质性病变引起的癫痫或长期严重的癫痫发作及大量滥用药物造成的癫痫

★智能障碍：言语不清或言语表达能力与其年龄不相符，对于人、物品、时间、地点的识别能力低下，记忆、理解及计算能力与其年龄及文化程度不符合。

★皮肤异常：许多先天性或遗传性神经疾病合并癫痫发作的病人常有皮肤异常。脑面血管瘤病(Sturge-Weber综合征)在一侧面部三叉神经分布区可见片状的红色血管痣。结节性硬化症，可于鼻翼两侧及面颊部见到大小不等的浅红色或鱼肉色的皮脂腺瘤，全身可见多处散在的色素脱失斑。苯丙酮尿症病人的皮肤较白，毛发呈黄褐色。

★头颅异常：头围过小，见于脑小畸形、脑萎缩、颅缝早闭;头围过大见于脑积水、脑肿瘤、慢性硬膜下血肿及某些神经系统变性病。1岁半以前的儿童前囟饱满膨隆、紧张，多有颅内压增高。颅内压增高时可使幼儿已经闭合的颅缝重新裂开。这时叩击头部可听到破壶声。有颅内动脉瘤、动静脉畸形或快速生长的胂瘤时，于头部可听到血管杂音。婴幼儿有硬膜下积液、硬膜下血肿、脑积水、脑萎缩，颅透照试验可发现异常透光带。

★五官异常：小眼球见于先天性风疹或弓形体感染及某些染色体病。白内障见于先天性风疹综合征、先天性弓形体感染及半乳糖血症。两眼内眦距过宽多见于先天愚型。18-三体综合征可有耳畸形及位低。脆性X染色体综合征可见耳过大。舌大而厚见于呆小病、黏多糖病。舌宽平见于先天愚型。

★皮纹异常：不少先天异常合并癫痫病人有皮纹异常。21-三体综合征常有腕部三叉线高位、通贯掌等。正常人指纹箕向桡侧开口的多见于第二指，如癫痫病人其它指出现桡侧箕形;则可能有染色体异常。

★神经系统异常：颅内感染、占位性病变、血管病变、中毒、外伤等，除有癫痫发作外，可有颅内压增高;眼底可有视乳头水肿、出血、渗出等改变;一侧或双侧瘫痪，肌张力过高，腱反射亢进，出现病理反射，浅反射减弱或消失，复合感觉障碍，往往提示一侧或双侧大脑皮质有器质性损害。脑膜刺激征阳性提示颅内出血或感染。癫痫病人出现走路不稳、指鼻不准、眼球震颤。各种抗癫痫药中毒时都可以出现意识障碍，轻者嗜睡，重者昏迷。
