2、突然跌倒、意识丧失、不发生抽搐的患者，应注意观察和询问发作前有无心慌、胸闷、出冷汗，脉搏是否过快、过慢，规则、强弱如何，血压高低，发病是餐后还是饥饿状态，全身是僵硬的还是柔软的，这些有利于鉴别是晕厥还是癫痫。这是癫痫患者发作时的症状之一。

3、癫痫大发作时抽搐是双侧同时开始或自哪一侧开始，哪一侧或哪一个肢体抽搐较重，持续时间较长，发作时头、眼有无向一侧歪，口角是否向一侧歪，这些可能提示癫痫放电的起始部位，也就是病灶部位。

4、很多的患者都是突然停止活动，呼之不应，双眼发呆，伴有咀嚼、吞咽、咂嘴等动作，或伴有面部及双上肢轻微抖动时，要注意记录发作持续的准确时间，发作过后询问患者发作前及发作后有何不适感，是否知道有发作。

5、患者发作的时间和地点是白天还是夜晚，夜晚发作是睡眠中发作，还是未入睡发作，是在有人的场合发作，还是无人在场发作，这些有利于鉴别是癫痫还是癔病发作。

6、对癫痫大发作患者要注意观察有无先兆症状，如患者开始抽搐前有无幻觉或错觉，有无情感反应等，并应注意其表现。
