小儿高热惊厥如何治疗

1.控制惊厥

(1)止惊安定为止惊首选药物，0.3~0.5mg/kg/次，最大用量不超过10mg，静脉注射或肌肉注射，静脉注射速度需大于1min。也可用苯巴比妥钠(鲁米钠)5~10mg/kg/次，静脉注射。5分钟后抽搐仍未控制可给咪唑安定0.1~0.2mg/kg/次，0.1~0.2mg/kg.h持续静脉输入。

(2)脱水剂应用严重反复惊厥或惊厥持续状态常继发脑水肿，宜加用脱水剂。20%甘露醇0.5-1g/kg.次，静脉注射q6h，q8h应用1-2天。严重者加用速尿1mg/kg.次和地塞米松0.5mg/kg.次，静脉注射1-2天。

2.一般治疗

(1)取侧卧位，松解衣服及领口，清除口腔、鼻、咽、喉部分泌物及呕吐物，以保持呼呼吸道通畅。头偏向一侧，防止吸入窒息，在上下牙处安放牙垫，以防舌咬伤。

(2)吸氧和心电监护。

(3)降温等对症处理。

(4)纠正水、电解质及酸碱平衡紊乱。

3.病因治疗

(1)感染应用抗生素控制感染。

(2)低血钙10%葡萄糖酸钙2ml/kg加入10%葡萄糖50ml，静脉滴注，并同时听心音，有心动过缓、心律失常时应停止注射。

(3)低镁血症钙剂应用止惊无效或血电解质检查有低血镁者，25%硫酸镁0.2-0.4ml/kg.次，深部肌肉注射4次以上，5天为1个疗程。亦可用1%硫酸镁5ml/kg。

(4)低血糖10%葡萄糖溶液10-20ml静脉注射。

(5)维生素B6缺乏症维生素B650-100mg加入液体静脉滴注。

(6)脑性脚气病(维生素B1缺乏)维生素B1100mg/次，肌肉注射。

(7)其他如破伤风者应使用破伤风抗毒素，脑炎、脑膜炎和颅内占位、代谢性疾病等应尽早查明原因，作针对性治疗。

4.热惊厥治疗

止惊、退热、治疗原发病。降温治疗虽不能防止热惊厥复发，但可防止脱水，改善症状。
