有些过去认为找不到病因的癫痫，由于近代诊断手段的不断进步而不发现了明确的病因。例如大脑灰质异位，在19世纪病理检查已发现了，并明确为患者生前癫痫的病因。灰质异位是大脑很微小的发育畸形，所谓灰质一般都在大脑表面由神经细胞组成，而异位的灰质则在大脑深部。20世纪80年代磁共振成像(MRI)应用于临床以后在生前即可发现灰质异位，而属于症状性癫痫。

3.继发性癫痫:

继发性癫痫亦称症状性癫痫，是由于其他疾病导致的癫痫发作，癫痫发作是原发病的一个症状，如脑的局部或弥散性病变，全身性代谢或中毒性病变等。其中颅内疾病继发的癫痫病最常见。所以今后随着检查手段的进步，肯定原发性和隐源性的范畴会越来越少。
