目前认为原发性癫痫致病因素与遗传因素有较密切的关系，或有家族性出现的倾向。

原发性癫痫亦称特发性癫痫，在这类病人的脑部并无可以导致症状的结构变化或代谢性疾病表现，目前认为原发性癫痫致病因素与遗传因素有较密切的关系，或有家族性出现的倾向。国内外大量的癫痫家系和流行病学调查发现，普通人群的癫痫患病率为0.5%～1%，原发性癫痫的亲属癫痫患病率为19.8%，个别高达69%，继发性癫痫的阳性家族史为1%～4.5%。癫痫通过何种方式进行遗传，部分学者提出多基因遗传的观点，认为致病基因的作用必须在许多基因积累效应共同作用的基础上发病，且与外界环境因素关系密切，即癫痫发病既受遗传因素的影响，又与许多促发因素有关。

有关研究认为父母近亲、发热惊厥家族史、精神分裂及偏头痛家族史都有较高的癫痫发病率，且一般不表现为显性或隐性遗传。另外在临床中也有许多和先天性中枢神经系统疾病或心脏畸形合并癫痫的病例，如结节性硬化、神经纤维瘤、家族性黑矇性痴呆、异染性脑白质营养不良等并不符合任何一种孟德尔遗传，因而应归于多基因遗传。
