目前认为：大脑皮质、灰质、脑干、小脑和脊髓等的不同平面均可引起肌阵挛，其中以脑干下段的网状结构，特别是延脑的巨细胞核放电在发病中的意义最大，另外，也可能是继发于脑部的疾病，如神经系统变性、脑外伤、感染等。
