1.人种差异影响癫痫发作

突变c.473+5G>A并非首次报道，2009年Depienne等曾于一例白人DS患儿上发现了同样的突变;而迄今为止SCN1A剪切位点突变出现较轻表型的情况只在亚洲人种中出现过，提示不同人种的遗传背景差异可能对SCN1A剪切位点突变的表型存在影响。

2.环境因素影响癫痫发作

生物的表型是环境与基因型相互作用的结果，基因型完全相同的生物体在不同的生长条件下可能对基因表达、转录及加工等过程产生影响，改变最终产物，产生不同的表型。环境因素在SCN1A基因的表达过程中可能发挥了重要作用。

3.药物和治疗因素影响癫痫发作

早期研究发现在新诊断的癫痫患者中，选择不同类型抗癫痫药物可以对患者的表型产生一定影响;同时较好的控制发作，对改善患儿认知功能和体征亦有帮助。由于钠通道剂拮抗类抗癫痫药物可能加重PEFS+患者发作，本癫痫中心尽量避免对这类患者使用以上药物使多数患者发作得到良好控制，这也可能是464号患者临床表型明显较轻的原因。

4.基因间的相互作用影响癫痫发作

同时敲除Scn1a和Scn8a的小鼠较仅敲除Scn1a的小鼠表型更轻，因此认为SCN8A基因可能对SCN1A突变患者表型产生了影响。但此观点还需在患者中进一步证实。通过本实验，本院初步了解了SCN1A剪切位点突变的致病机制，证明了突变c.473+5G>A可通过引起Nav1.1截短并丧失功能而致病，但患者出现较轻表型而非严重表型的原因还有待更深入的研究。
