癫痫大发作时，病人表现为腿部痉挛抽搐，头部后仰，大叫一声后摔倒在地，全身肌肉呈强直性收缩、痉挛，嘴巴紧闭，两眼上翻，僵直期一般持续数秒至半分钟，转为阵挛期，此期全身肌肉呈有节律的强烈收缩，呼吸恢复，随呼吸口中喷出白沫或血沫，尿失禁等，一次发作持续2-3分钟，多的可达7-8分钟。当病人发生全身抽搐前将要倒地时，患者家属或救助者若在附近，要立即上前扶住病人，尽量让其慢慢倒下，以免跌伤。
