痴笑性癫痫是怎么引起的

痴笑性癫痫的致病源是下丘脑错构瘤。其主要表现为部分性癫痫，为自主神经症状的发作，表现为发作性傻笑，持续数秒或数十秒而突然停止，发作时无神志丧失，每日可发作数十次，无任何诱因。随病情发展，可逐渐出现其他类型的癫痫。

下丘脑错构瘤又称灰结节错构瘤，为临床罕见的颅内先天性畸形，多发于儿童，主要表现为体内雌激素水平过高、第二性征发育早、骨龄增加及伴有无诱因的狂笑发作。有人认为下丘脑错构瘤起源于乳头体或灰结节，于妊娠第35-40天形成小球脑板时错位所致，是一种中线神经管闭合不全综合征，由正常脑组织所形成的异位肿块，组成此种畸形的神经细胞类似于灰结节中的神经组织，并伴有正常胶质细胞。下丘脑错构瘤并非真正的肿瘤，不具有生长性。下丘脑错构瘤该病严重影响儿童身体的正常发育。由于过去对该病发现率极低，常造成误诊或漏诊。

随诊病人，从影像学、病理学、临床体征等多方面，对该病的发病特点及规律进行了较为系统的研究。经电镜观察他们首次发现：下丘脑错构瘤神经元有部分细胞核变异;神经毡及突触过于密集，并有神经分泌颗粒存在。错构瘤在结构上与大脑边缘系统有异常密切的关系，是各类型癫痫的致痫源;根据胞浆及轴突内存在大量的神经分泌颗粒(部分含有GnRH)，证明其为独立于下丘脑至垂体轴的内分泌单位，是导致性早熟的根本原因。

痴笑性癫痫非发作期行为和正常人一样。有的病人在发作前表现出突如其来的惊恐样：全身活动静止，双目凝视，意识模糊，颜面潮红，旋即咯咯大笑起来，有时甚至笑得大小便失禁。
