原发性癫痫是指通过现有手段不能明确原因的病例。继发性癫痫是指通过目前检查手段能够发现确切原因或脑部实质病变者。

儿童脑外伤癫痫属于继发性癫痫，但孩子外伤后并不是100%都发生癫痫，儿童脑外伤癫痫的治疗应视病情及脑电图情况而定，如伤势较轻，脑CT或核磁正常，脑电图正常，无癫痫放电，可严密观察，暂不用抗癫痫药。

孩子外伤后一旦有癫痫发作，应及早抗癫痫治疗，儿童脑外伤癫痫的治疗越早效果越好。脑外伤后病情较重者，或脑电图不正常，即使无癫痫发作，亦应给予预防性抗癫痫药治疗，查脑电图基本正常，可减量至停用。脑外伤后合并癫痫者，一定要按癫痫发作类型不同，选用不同的抗癫痫药物治疗。

儿童癫痫诱发因素较多，有些癫痫是由于以下原因引起：

中枢神经系统的感染、脑发育畸形、各种脑缺氧缺血性疾患、代谢紊乱、中毒等。

家长要注意作好围产期保健、产前检查、优生优育并按免疫程序接种疫苗预防流行性脑膜炎、病毒性脑炎、结核感染等。平时，要谨防孩子脑外伤。

儿童脑外伤癫痫的治疗宜从单药治疗开始，长期规律服药，减药过程要慢，按发作类型用药。
