失神伴植物神经症状失神发作时常可观察到植物神经的症状，如瞳孔扩大，面色苍白或潮红，心动过速，呼吸改变等，少数可有尿失禁。

失神伴轻微阵挛性成分见于半数左右的失神发作，主要表现为失神发作时伴有面部或上肢轻微的肌阵挛性抽动。如肌阵挛为突出的症状，则应考虑为肌阵挛性失神。

失神伴失张力成分约占失神发作的20%左右。发作时维持姿势的肌肉张力减低，通常表现为头部缓慢下垂，但很少因肌张力完全消失而致跌倒。失神伴跌倒发作一般见于不典型失神。

失神伴强直成分主要表现为失神发作时姿势性张力轻度增加，以影响伸肌为主，最常累及眼肌，引起眼球向上凝视。累及范围可进一步扩大到颈部或躯干，导致头向后仰或躯干的后冲性运动。不对称的姿势性强直可导致头或躯干转向一侧。有时强直中伴有轻度的阵挛成分。
