导致癫痫的因素及影响

(1)遗传因素：目前人们已经认识到癫痫与遗传有关，而且对儿童癫痫的影响更大。具体表现为不但癫痫本身有遗传倾向，而且对一些其他遗传性疾病来讲，癫痫症状也是经常见到的，甚至可能为主要症状，如苯丙酮尿症、神经纤维瘤病、结节硬化等。临床资料证明对有遗传倾向的癫痫患儿，遇到各种一般的、轻微的外因时，就可引起发作。即使不出现明显的临床发作，也可以证明有发作阈值降低，即脑电图出现棘波或多棘慢波放电。

(2)获得因素：即脑部有癫痫性病理改变，这些获得性脑损伤引起脑的结构性或代谢病变，产生癫痫灶，病变可以是弥慢性或局限性、静止性或进行性。脑损伤的原因很多，但在小儿多为先天发育异常、产伤或外伤、脑部感染、高热惊厥等。

(3)性别、发病年龄及发作类型：一般男多于女，以幼儿最多，而且年龄与发作类型也有密切关系，如婴儿痉挛症几乎均在1岁以内，运动性发作发病在6岁内，失神发作发病多在1～8岁，其他各型癫痫的首发年龄也多在10岁以内。新生儿癫痫往往隐蔽，且呈局灶发作，年长儿抽搐明显且呈全身性。

(4)发作表现：癫痫各发作类型的发生率随年龄而不同，其临床表现与中枢神经系统的成熟程度密切相关。除常见的发生于儿童任何年龄的大发作外，新生儿癫痫其临床表现形式多为刻板的反复性动作，并常伴有异常的眼球运动。

(5)智力改变：凡有明显脑器质性、遗传性、代谢性病因者，以及有神经系统异常体征者，几乎均有智能低下。年龄越小，智能障碍的发生率越高，而不同发作类型伴智力下降者，由高到低，依次为婴儿痉挛、失神发作，而且发作越频繁，智能低下率就越高，严重发作本身即可影响智力发育。

除此之外，小儿癫痫还有在同一病人身上发作的多样性、易变性和不良因素容易诱发等特点。
