喹诺酮类使用可诱发癫痫的发作，对于已知或怀疑有可能导致癫痫发作或降低癫痫发作域值的中枢神经系统疾病的病人，莫西沙星在使用中要注意。由于临床试验数据有限，不推荐该药用于肝功能严重损伤的病人。莫西沙星象其它喹诺酮类和大环内酯类抗生素一样在有些患者可能引起QT间期延长。因为缺乏相关的临床资料，该药应避免用于QT间期延长的病人、患有低钾血症病人或接受Ia类(如：奎尼丁，普鲁卡因胺)或III类(如：胺碘酮，索托洛尔)抗心律失常药物治疗的病人，在使用莫西沙星时要慎重。莫西沙星与下列药合用不排除有延长QT间期的效应：西
